Generations contracts with a combination of licensed home care, home modification, and individual service providers in the six-county service area (Daviess, Dubois, Greene, Knox, Martin, and Pike). The services available to the individuals we serve and their families/caregivers include respite care, adult day services, home and community assistance, home health aide, attendant care, skilled nursing, therapies, home delivered meals, assistive technology, minor home modification, rehabilitation, emergency response systems, transportation, and home health supplies. The availability of a variety of home care agencies provides individuals the flexibility needed to ensure they are able to select the provider(s) that will best suit their personal needs. Case Managers and Options Counselors work with each individual using a person-centered approach throughout the provider selection process.

All agencies interested in providing services for individuals served by Generations must be certified through the Medicaid Waiver program or contracted with Generations to provide Non-Waiver Services. The Medicaid Waiver program is administered through the Division of Disability, Aging, and Rehabilitative Services (DDARS) and differs from traditional Medicaid certification.

How to become a Certified HCBS Waiver Provider

The FSSA Office of Medicaid Policy and Planning (OMPP) certifies providers for home and community-based services (HCBS) through the Indiana PathWays for Aging, Health & Wellness (H&W), and Traumatic Brain Injury (TBI) waivers. These waivers allow individuals to receive services in an individual’s home or other community setting rather than an institutional setting.

Providers who wish to participate in one of the waiver programs will need to complete three steps:

  1. Become certified by OMPP to provide HCBS waiver services

  2. Enroll with the Indiana Health Coverage Programs (IHCP)

  3. Finish your waiver program enrollment

How to become a Non Waiver Provider

Generations is the Area 13 Agency on Aging, serving Daviess, Dubois, Greene, Knox, Martin, and Pike counties in Indiana.

To partner with our agency as a Non Waiver provider so that we may offer more choices to the individuals we serve, please review the Checklist for New Providers, Vendor Memorandum of Agreement, Attachment A, and Vendor Contact Specification below. If all terms are agreeable, please contact us, and DocuSign documents will be sent for completion. The DocuSign packet and all additional required documentation must be submitted and VU onboarding complete before a provider is added to the service picklist.  

As a provider you are subject to periodic Quality Assurance Compliance Reviews by our agency. A copy of Compliance Review Tool is below.
