Please submit your request no later than the close of the previous business day before
your appointment, to give us time to respond to you. A request for a Monday appointment
must be submitted by the close of business on the previous Friday.
Test appointments can be set only between 9 a.m. and 2 p.m. Please do not request
a time earlier than 9 a.m. or any later than 2 p.m. Be sure to include the time as
well as the date. We cannot confirm appointments with no time specified.
If there is no time limit, give a generous estimate of the time you expect the exam
will require.
Note: This form is for use by Vincennes University students or students enrolled
at other colleges or universities who require proctor services for testing at the
main campus Testing Center in Vincennes. If you are a new student and need to take
the placement test, see the webpage for Accuplacer placement testing and do not use
this form.
